So let us put the plain version first. Behind the bladder, wrapped around the tube the urine travels down, sits the prostate. Lying down changes where the fluid in your body sits. During the day gravity keeps some of it in your legs; at night it returns to circulation and your kidneys clear it. In a younger man the bladder holds that volume until morning. In a man whose bladder is already only half-emptying, it does not.

A urologist starts most men on tamsulosin, which relaxes the muscle at the bladder neck and works inside a week; it costs around Rs 1,400 a month and you stay on it. Finasteride and dutasteride go after DHT over a year rather than a week, and bring sexual side effects that send a lot of men looking for an alternative.
